I agree that I have completed the training and will follow the program fidelity of the class curriculum I am leading in accordance with the license; that I will keep the class status updated and will submit the participant attendance and survey forms as required. I understand that once a year I will receive an email asking me to verify that I still want to belong to the NDC3 system. If I do not verify according to procedures I understand my status will be changed to inactive.
