Better Choices, Better Health: Chronic Conditions

This program is designed to help participants deal with chronic conditions. Topics include: techniques to deal with frustration, fatigue, pain and isolation, appropriate exercise for maintaining and improving strength, flexibility, and endurance, appropriate use of medications, communicating effectively with family, friends, and health professionals, nutrition, and how to evaluate new treatments. People with chronic conditions and their caregivers can participate in this program.

Program Overview

Better Choices, Better Health: Chronic Conditions is a group workshop that meets online (virtually) 1 time a week for 6 weeks. A trained peer leader guides each session.

This program focuses on learning skills and supporting one another with chronic health conditions.

Key Learning Objectives
  • Discuss healthy eating basics.
  • Learn ways to manage stress and emotions.
  • Discover tips for better sleep and staying active.
  • Apply breathing and relaxation skills.
  • Learn ways to talk with family, friends, and doctors to make health care decisions.
  • Describe safe use of medicines.
Who should attend?

Adults 18 or older, or their support person, living with one or more long-term physical or mental health conditions.

Program Schedule / Format

This program meets online for 1 hour a week for 6 weeks in a group setting with discussion and activities.
